Insurance and Payment
Our treatments vary in scope and complexity, so it is difficult to provide a price list for our services. However, all costs and fees will be clearly explained during your initial consultation appointment.
You must have your insurance information at the time you make an appointment. Please bring your complete insurance information, including your insurance card, to the appointment. Many insurance companies will pay for a portion of your treatment. However, most insurance types do not provide 100 percent coverage. We collect your estimated copay portion.
We are a preferred provider for Delta Dental and LLUH Risk Management.
We accept all indemnity insurance.
Payment is due at the time that service is rendered.
We accept payment through Flex Cards, cash, checks,Care Credit, and all major credit cards.
