Fees vary based on the work needed and treatment options. All fees and costs will be discussed in detail at your consultation appointment and prior to any treatment.

Many insurances will pay for a portion of your treatment. However, most coverage is not 100%.

We will need your insurance information at the time you make an appointment. Please bring your complete insurance information, including your insurance card, to the appointment.

We are a preferred provider for Delta Dental (PPO & Premier) and Loma Linda Risk Management.

We accept all indemnity insurance.

Payment is due at the time service is rendered.

Yes . Please arrive 30 minutes prior to your scheduled appointment time. Also bring your referral form and your medication list.
No. Since you are a previous patient and have a dental record with us, we can schedule an appointment for you without another referral.
All of our doctors are competent and qualified full-time instructors. Appointment availability changes according to quarterly teaching schedules. We will try our best to schedule you with the specialist your dentist referred you to.

Faculty Endodontics has reserved parking for our patients. The referral slip shows the parking area.

When driving from the freeway, going south on Anderson Street turn west (right) on Taylor Street. HALF WAY down the block there is a small parking lot on the right with a tall palm tree at the entrance.  When turning into the lot you will see a building that has a sign on it saying "Research".  Our parking spaces are on the right as you enter the lot they face the University Arts Building across the street.  From the street the back of the signs are bright green.  Exit your car and continue walking west to the end of the block, our office is at the end of the block on the first floor 24860 Taylor street.  Call us if you are lost: (909)558-4990. No. Our office is limited to the specialty of endodontics.
No, unless special arrangements are made. Our office hours are 8:00 a.m. to 5:00 p.m. Monday through Thursday and 8:00 a.m. to noon on Friday. We are closed on Saturday and Sunday. We have some early evening appointments available for treatments only. Consultations are scheduled only during standard office hours.

We work with a network of experienced and caring dentists. We recommend you return to the dentist who referred you to our office.

If you are not seeing a dentist currently, and are a self-referred patient, we can provide you with names of dentists we associate with in your area. We do not endorse any single dentist or office.
